NEW PATIENT REGISTRATION
Patients wishing to register with Hawkhill must complete
a registration form, a simple medical questionnaire and have a routine
medical health check. This will assist your doctor to care for you
until your records are transferred from your previous doctor.
Patients are registered with the Practice rather than an individual
doctor but retain the right to see the doctor of their choice.
The Practice has an agreed Practice Area which is shown below.
We will only accept patients who live within this boundary.
Existing patients who live outwith the Practice area will NOT be
asked to Leave, but if existing patients move to an address outwith
the boundary then they will be asked to find a surgery nearer their
home. This policy applies to patients who live inside or outside
the boundary at present.

SUGGESTIONS, COMPLAINTS AND QUERIES
If you have any suggestions, complaints or queries regarding the
services provided at Hawkhill Medical Centre please speak to one
of the receptionists or write a note for the Practice Manager, Mrs
Elaine Hendry.
Any comments you may have would be most welcome.
In the case of complaints we would like to be informed of these
as soon as the need arises. Please ask to speak to Mrs. Hendry,
or a doctor if you prefer, so that the problem can be investigated
and resolved, to everyone's satisfaction, without delay.
You may also put the complaint in writing if you prefer.
The Receptionist will give you a copy of the Practice Complaints
Procedure on request.
POLICY FOR CONSENT TO THE TREATMENT OF CHILDREN
All Clinical and Administrative staff at Hawkhill Medical
Centre are aware of their responsibilities in protecting the rights
and confidentiality of children and young people.
As stated in Section 2(4) of the Age of Legal capacity (Scotland)
Act 1991 provides that "A person under the age of 16 years
shall have legal capacity to consent on his or her own behalf to
any surgical, medical or dental procedure or treatment, including
psychological or psychiatric examination, where, in the opinion
of an attending qualified medical practitioner, he or she is capable
of understanding the nature and possible consequences of the procedure
or treatment."
FORMS (PRIVATE INSURANCE FORMS, INCOME PROTECTION ETC)
Please fill in the appropriate details i.e. your name,
address, dates of absence/illness etc before handing the form in
to reception. It may take up to 7 working days to complete these
forms and there will be a charge to cover administration costs.
NON NHS MEDICAL EXAMINATIONS
Examinations for special purpose (e.g. Insurance, fitness
to undertake sports, HGV, PSV etc) do not come under the National
Health Service and require a longer appointment. The receptionist
will be happy to arrange an appointment with the doctor of your
choice. There is a fee for this service as recommended by the British
Medical Association.
DISCHARGE FROM HOSPITAL
If you spend a period of time in hospital with a serious
illness, having had an operation or having a baby, please let the
Practice know when you have been discharged home as it can take
a number of days for hospital correspondence to reach us.
YOUR DOCTOR
Doctors need to make notes about any diagnosis, test results,
treatments including drugs, prescriptions and other information
that you may provide, that seems relevant to the treatment of your
condition.
We need to keep this information in order to provide proper care
for you (for later treatment, or if you should be seen by another
doctor) and to allow others to check the treatment that you have
received. Nurses and other professionals also need access to these
records, and will add their own notes, as part of the overall healthcare
provision. Secretaries, receptionists and other clerical staff need
access to some of your records in order to do administrative tasks,
such as: booking appointments and communicating with you and other
parts of the NHS.
THE HEALTH SERVICE
In order to manage the NHS, some restricted information
concerning treatments, drugs prescribed, numbers of patients seen
etc. is needed and hospitals and general practices must provide
this information in returns to various central bodies. This information
has personal details such as your name and address removed wherever
possible.
OTHER AGENCIES
The NHS is not the only government service to provide you
with care, and it will be necessary for us to provide other agencies
with appropriate information, but only with your consent (or that
of your relatives if you are too ill).
FREEDOM OF INFORMATION ACT
The Freedom of Information (Scotland) Act 2002 gives the
public a right of access to all types of recorded information by
public authorities, including the Health Service. We are obliged
to respond to requests for information and provide that information,
subject to particular conditions and exemptions set out in the Act.
The Practice has adopted the British Medical Association Model Publication
Scheme to meet the requirements of the Scottish legislation. Much
of the information covered by the scheme is contained in this booklet
but should you require further information please contact the Practice
Manager.
INFORMATION FOR HEALTH
We ask you for information so that you can receive proper
care and treatment.
We keep this information, together with details of your care, because
it may be needed if we see you again.
We may use some of this information for other reasons.
HOW WE PROTECT YOUR INFORMATION
The sensitivity of patient information is well understood
within the NHS. All staff and contractors are trained to respect
their duty of confidentiality to you and have this written in their
contracts.
HOW CAN YOU FIND OUT WHAT'S IN YOUR MEDICAL RECORDS
We are required by law to allow you access to your medical
records. All requests to view medical records should be made in
writing to the Practice Manager. Request forms are available at
reception. We are allowed by law to charge a fee to cover our administration
and costs.
To protect your privacy and confidentiality we will not normally
disclose any medical information to your family, friends, or colleagues
about any medical matters at all, unless we know that we have your
consent to do so.
If you have any further queries, comments or complaints about privacy
and your medical records please contact the Practice Manager or
talk to one of the doctors.

VIOLENCE OR ABUSE
THe Practice considers aggressive behaviour to be any personal,
abusive and aggressive comments, cursing and / or swearing, physical
contact and aggressive gestures. No abuse of staff or doctors is
acceptable whether verbal or physical and any such incidents will
result in removal from our list.
