Applications to join our practice list will be for the following only:
- Patients who have been removed from their current Practice list, as they are now residing out with their catchment area (evidence required)
- New babies of existing patients
- Dependents of patients already registered with us
- Temporary Residents in our catchment area, visiting ONLY for less than three months e.g. on holiday
- If you are residing here for more than three months you will require to register with a GP permanently

Patients living within the practice catchment area who do not fall in to the categories above should refer to the NHS Inform GP Finder to locate a practice in their area.
Patients wishing to register with Hawkhill Medical Centre and who live within the Practice boundary, will require to complete a new patient registration form. New patients will also be asked to complete a new patient questionnaire form which will assist the doctor to treat you until your medical records are received from your previous Practice.

Acceptance on to the list is as at the discretion of the Doctor and we may ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.
Patients are registered with the Practice rather than a named Doctor but retain the right to see a doctor of their choice. New patients who require repeat medication will have to make a GP appointment before a prescription will be issued.
Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.
Student Registration Guidance
Welcome to Hawkhill Medical Centre, we are pleased that you have chosen to register with our Practice and wish you all the best in your studies.
All students are advised to register with a doctor during their time in Dundee. If you are here for less than three months, you can register as a temporary patient if you require medical assistance.
Please complete the online forms and submit your identification along with proof of address. We cannot complete your registration until we receive both forms.
Please ensure that you complete all fields including:
- “Home Address” box is for your Dundee address
- “Address in UK when last registered with a GP” box is your address back home
- “Name & address of previous GP Practice” box allows us to obtain your medical records.
Please do not complete these forms until you are physically residing in Dundee. You may be deducted from your current GP list too early.
The Practice may also contact you via email/text message for appointment reminders. Please tick the appropriate box within the Communication Preference section. Please allow 4 working days for your registration forms to be processed.
If you require urgent medication or medical assistance please contact the Practice on 01382 669589.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
It is is important to note that you can only permanently register with a GP practice if you’re staying in the area for more than 3 months.

Step 1: Complete online registration form
Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.
Step 2: Upload scans of photo identity and proof of address
Please deliver to the practice copies of two forms of your identity.
One form of identity must contain a photo of yourself and the other must provide evidence of your current address. You may upload your forms online.
